inconsistent with UIPカテゴリーの7所見のうち、ground-glass opacity, air-trapping, consolidation, axial distributionは病理学的non-UIPと関連が高かったが、upper-mid lung predominanceは病理学的non-UIPと関連がなかった。

現在のCT分類は十分ではなく、特に"inconsistent with UIP"の名称は不適切。

では、Lancet Respir Med 2018年4月号に掲載されたDYNAGITO試験について。

Design: Double-blind, randomised, parallel-group, active-controlled trial
P: COPD with a history of exacerbations
I: tiotropium-olodaterol 5 μg-5 μg
C: tiotropium 5 μg once daily
*Patients using inhaled corticosteroids continued this therapy
O: Rate of moderate and severe COPD exacerbations
*From the first dose of medication until 1 day after last drug administration
・9009 patients were screened from 818 centres in 51 countries
 →Recruited 7880 patients (日本人461例) 
  mean age 66・4 years [SD 8・5]
  5626 [71%] were men
  mean FEV1% predicted 44・5% [SD 27・7]
 →3939 received tio-olo vs 3941 tiotropium
・The rate of moderate and severe exacerbations
  tio-olo 0.90 ≦ tiotropium 0.97
 *Rate ratio (RR): 0.93, 99%CI 0.85-1.02; p=0.0498
 →Not meeting the targeted 0.01 significance level
・Adverse events was similar between treatments.

・Tio-olo did not reduce exacerbation rate as much as expected compared with tiotropium alone.
・However, there were larger improvements in some subgroups of patients, such as those receiving triple therapy at baseline, and in exacerbations that required oral corticosteroids.

・10 pack-years以上の喫煙歴→〇
 もしくは両者を要する. 入院の有無は問わない)→〇

<Exclusion criteria>
 {ベンザルコニウム塩化物(BAC), EDTAなど}に過敏症あり



<triple theapy群の患者で増悪抑制効果が高い>
・ベースラインtriple therapy群では、増悪抑制効果は
  tio-olo>tio(Rate ratio 0.847, 95%CI 0.766-0.936)
 ➤ triple therapyは日本でこんなにいないでしょうし、
  そのような患者さんをあえてstep downしないでしょう

・これらの患者群では年間増悪回数は tio-olo ≒ tio
 (0.97 vs 1.07, Rate ratio 0.911, 95%CI 0.841-0.988)
 ➤triple therapy群が引っ張っているのでしょう

 tio-olo ≧ tio(0.47 vs 0.62, RR 0.757, 95%CI 0.523-1.095)で惜しい

・tio-olo 12.4% vs tio 16.5%

・triple therapyが入っている場合、tio-ICSにステップダウンはダメ
